When you request an infusion appointment, we collect:
- the patient’s first and last name and age
- a parent or guardian’s name, if the patient is under 16
- a mobile number and, if you give one, an email address
- the type of infusion requested, whether the patient has a valid Medicare card, and a preferred date and time
- any documents you email us, such as a referral, prescription or test results
Why we collect it
We use these details to contact you about your request, arrange your appointment, and prepare for your visit. We don’t use them for marketing, and we don’t sell them.
Who can see it
Requests go to SWIFT’s clinic team by email. Our email provider [Google Workspace] and the service that delivers the form [Cloudflare] handle the details only to deliver them to us. [SWIFT to confirm whether data is stored outside Australia.]
Website analytics
We use Google Analytics and Google Tag Manager to understand how people use this website, for example which pages are visited and whether a request was sent. These tools use cookies. How long we keep it
